A patient named Becky set Mark Cacciamani, MD on his path. While practicing at a high-volume academic clinic, he met a woman whose motorized wheelchair physically could not fit through his exam room door, requiring three staff members and her entire appointment just to reach the table. Learning she lived five blocks away, he chose to see her at home. When leadership told him the system would not allow house calls, Dr. Cacciamani reached a crossroads and decided to build a new model rather than serve a broken one. Today he is Owner and Chief Executive Officer of House Call Doctors and The Doctor’s Office, leading with a simple creed: Build systems that let great people do their best work, so patients feel the difference.

Leaving the security of the university, he took over a small, struggling house-call practice and quickly recognized that mobile care needed a sustainable engine. That insight transformed him from Physician into Business Strategist. He mastered the complexities of value-based care, became a Certified Risk Adjustment Coder and developed proprietary smart-form technology that subsidized his primary care mission. The diversified revenue strategy carried the practice from a single local service into a multi-state enterprise that evaluated more than 9,000 patients across four states in the past year.

His expansion has been deliberate and integrated. Beyond Utah, the in-home assessment model now reaches Nevada, Colorado and Idaho, while a partnership with a chiropractic and wellness clinic in Sandy City blends medical care, rehabilitation and wellness under one roof. A natural extension of the mission is The Manor on 1st, a 54-room assisted living facility he is building from the ground up to close the gaps in custodial care he witnessed for over a decade, aligning medical oversight directly with residential living.

Named to the 2026 Salt Lake City Titan 100, Dr. Cacciamani describes himself as an architect of integrated care, defined by systemic disruption, financial innovation and the courage to build. Where most providers adapt care to fit the four walls of a clinic, he forced the system to adapt to the patient, expanding access for the most complex and underserved populations in the Intermountain West. He secured direct contracts with the region’s largest insurers and proved that high-touch medicine can also be highly profitable.

Dr. Cacciamani’s passion stays lit through three practices: continuous learning, embracing new challenges and investing deeply in relationships. Hearing patients’ stories grounds him in why the work matters, while supporting his team keeps enthusiasm contagious. He sees artificial intelligence as a multiplier, already easing documentation burdens and poised to strengthen coding, billing and population health management.

Perhaps his proudest achievement is cultural. Dr. Cacciamani has built a workplace where clinicians are freed from the worried-well treadmill and empowered to practice high-impact medicine, proving that compassion can be a competitive advantage.
